Customer Information
: Gather customer information for future marketing and warranty (in case providing warranty)

Fill the ‘Mobile Number’

2. Fill the ‘First Name’ (Mandatory field)
3. Fill the ‘Last Name’ and ‘Email Address’ (Optional field)
4.
Tick the radio box next to ‘SMS’
and ‘E-mail’ if you want to send notification (Optional)

Device Information
: Gather information for the device to make a booking for a specific device
1.
Select ‘Non-Warranty’ or ‘Warranty’
- ‘Non-Warranty’ : the booking is made for a new device or not for warranty

‘Warranty’ : customer had a
repair before and need to provide warranty for existing customer

Select Manufacturer from the drop-down
list (Mandatory field)
: Store owner or manager can set up the list of manufacturer on ‘Settings’ (Refer to…)
3. Fill the ‘Model Name’ (Mandatory field)
4.
Fill the ‘Serial or IMEI’ (Optional
field)
: Useful to track the previous booking and support repairers finding previous booking for warranty in the future
5.
Fill the ‘Faulty Description’
(Mandatory field)
: Store owner or manager can set up the list of faulty description on ‘Settings’ (Refer to….), or able to write description manually

Test Server
(Note. This filed
will be shown as customer’s business name)

Fill the ‘Note for Office’
field (Cannot see from customers)
: Person in charge for repair can make a note for other employees
2. Select ‘Warranty Period’

Fill the ‘Quotation’
: Quote will be $0 if this field is empty
